Having originated in Brisbane, FCP has recently expanded onto the Sunshine Coast. With a dedicated team, we offer support services specific to the individual’s needs. Ensuring choice and control, FCP empowers individuals, promoting independence and encouraging autonomy. In conjunction with our participants, FCP helps to facilitate goal setting and as such creating care plans tailored specifically to the participant’s needs. Our Support Workers are dedicated, caring and supportive of your needs, helping to facilitate social and community connection. Case Managers ensure that you are connected with specialised supports and the appropriate professionals, specific to the individual’s needs. Our participants are at all times reminded of their choice and control and our aim is to provide the quality of life that we all deserve.
